Visitor pre-screening questionnaires at check-in

1 Sept 2026 5 min read

Not every visitor should be waved straight in. Some sites need a few questions answered first — a health declaration, a safety confirmation, an eligibility check — and doing that verbally at a busy desk is slow and inconsistent. A pre-screening questionnaire built into check-in makes it quick and uniform.

Where pre-screening earns its place

  • Health or safety declarations at plants and hospitals.
  • Eligibility or purpose checks before granting access to a zone.
  • Policy acknowledgements — devices, photography, confidentiality.
  • Anything that should gate entry rather than be asked afterwards.

Ask at the right moment

The questionnaire can travel with the pre-registration invite so it is done before arrival, or appear at the kiosk for walk-ins. Answers are captured against the specific visit, so there is a record — not a verbal exchange nobody logged.

Branch on the answers

The value is in what happens next. A flagged answer can route the visitor to a person, deny entry, or trigger an extra step — while a clean response lets them continue. That turns screening from a formality into an actual control.

Keep it minimal and private

Ask only what the visit genuinely needs, capture consent, and keep answers purpose-limited — the same discipline as the rest of DPDP-aligned visitor data. A questionnaire is not a licence to over-collect.
